Return Authorizations (RA’s)
• Click here for an RA Request Form. Please fill it out in its entirety and we will issue you an RA number. Once you receive the number from us please write it on the finished form you printed out (a printable version will display after you click ‘submit’ on the RA Request Form) AND on the outside of your box and send your item back to:

* Once an RA number or return shipping label is issued in writing (fax or email) or by phone, the item must be received by us within 10 days or repair, refund or exchange is forfeited.
